New Patients

Everyone deserves good, reliable, accessible and low-cost health care. However, Health for Friends can only see a very specific clientele at this time. If you meet the eligibility criteria, please bring the required documents to Health for Friends to schedule an appointment.

All new medical patients must fill out the Patient Application (pages 1 and 2), sign the Patient Contract, and submit the completed forms to Health for Friends before the first appointment can be made. You may pick up forms at Health for Friends or click on the links below to download them.

Medical Eligibility Criteria:

1. 18 years of age or older
2. Norman resident
3. At or below 125% of 2011 Federal Poverty Guidelines (see chart)
   
4. No Insurance
   
   • No private insurance, Medicaid, Medicare, veteran benefits, or tribal health services

Required Documents:

• Proof of Residency:
  
  1. Utility bill, or
  2. Recently received mail, or
  3. Notarized letter from friend/family member
• Proof of Income:
  
  1. Current pay stub / unemployment check stub
  2. Recent tax return / most recent W2
  3. Notarized letter from employer stating your income
  4. Notarized letter from friend/family stating that you are unemployed
